Flu Shot Clinic Coming to DCMS Oct. 24th:
Passport Health will visit DCMS on Wednesday Oct 24th to administer flu shots to students for any families that want their DCMS students to get their flu shots at school.

The students only are able to receive the flu shot. See attached flyers, accepted insurance plans, and consent form.

They will not have the mist for kids this year, only the shot. The FluMist is on production hold through the manufacture as of right now.

What to send to DCMS: The student consent form filled out with insurance info on it and a copy of the card. If someone chooses, they may pay cash, credit, or check for $28.00 for the flu shot. The clinic will be at DCMS on Wed, Oct 24th from 7:00--11:00am.  We will pull students from class when we are ready, and we will only be able to pull students that have dropped off their consent forms by 7:30am that morning.

Veteran's Day Assembly: Save the Date!

Please mark your calendars for our annual Veterans Day Assembly- Monday, November 12th at 7:30am.  See the attached invitation.

Veteran's Day Wall of Heroes:

We recognize that nearly every DCMS family has at least one Veteran but that all Veterans may not be able to attend the Veteran's Day assembly.   In order to honor each DCMS family, we will be creating a Wall of Heroes. See the attached "DCMS HERO" form. Please complete the form and include a scanned photo of the Veteran.  Hero forms can be turned in by Wednesday, November 1st to Mrs. Emmons (8th Grade) or emailed to TaylorEmmons@dcsok.org  OR to Mr. Cory  (7th Grade) or emailed to BrentCory@dcsok.org OR they may be returned to the office.  Please do not include an original photo, photos will not be returned.  Scan an image or make a copy of an image to include on the form.  You may turn in as many hero forms as you wish. We hope that every student will be able to submit at least one to honor a family member or family friend who has served or is currently serving in the U.S. Armed Forces.

Safe School Survey:

The DCMS Safe School Committee has created a survey to help guide the committee as we look as school safety issues. Please take a few moments to complete this survey with your student. Just click the link below. These answers will be anonymous and are a valuable way to gather information from our students and families. Thank you. Questions? Please contact Assistant Principal, Stephen Buck: stephenbuck@dcsok.org

We Want Students Involved with DCMS:
Looking for a way to help your middle schooler "get involved" and active in our school?  We have a variety of clubs to encourage student involvement.  See attached flyer! Want to learn more and not sure who to ask?  Please contact our Student Activities Coordinator, AJ Graffeo, to learn more:  ajgraffeo@dcsok.org

State Test Scores:
The Oklahoma Department of Education is now using an online system to deliver annual test scores to families.  See attached Oklahoma State Testing Program (OSTP) Parent Portal Letter.  The state has also provided one copy of student scores to each school site for families that would rather pick up a paper copy; however, this is the final year that the paper copy will be available.  Thus we encourage all families to learn how to use the new state portal to obtain your child's scores.  See attached letter for more details on how to view your child's scores from April 2018.
8th Grade Yearbook Recognition Ads:

8th grade families: It’s time to recognize the hard work of your middle schooler with a recognition ad in this year’s yearbook! There’s no better way to say congratulations for making it through middle school and good luck in high school than putting it in writing as an amazing surprise for your child when he/she opens the yearbook in May and for years to follow. See the attached form for more information, please follow the instructions closely, and direct any questions to Mr. Graffeo at ajgraffeo@dcsok.org. Please note, the yearbook staff works on strict deadlines; therefore, there will be no late submissions accepted after November 2nd.

Yearbook Name Edits:

Students---Do you have a nickname that you go by? Do you want that nickname in the yearbook instead of your given name? Grab a name change form from the front office, fill it out, have your parents sign-off on it, and turn it in to the front office by October 26th. You must do this every year for your nickname to be used in the yearbook. If you have any questions, ask Mr. Graffeo ajgraffeo@dcsok.org

Bake Sale Fridays:
On several Fridays throughout the year various groups will sponsor bake sales and/or donut sales at DCMS.  See attached calendar with our current schedule of Bake Sales for the year.
